Job Function Summary:

Involves all aspects of the medication use process, including procurement, prescribing, compounding, dispensing, storage of medications, administration and monitoring. Partners with physicians and other clinicians to provide treatments and advice to patients, records patient history relating to medication use and ensures the safe and secure storage of medications and related supplies. Advises physicians in determining the best possible treatments for specific diseases, stays current on pharmacy regulations, treatments and developments in research and design. Pharmacists are the experts on drug therapy and the primary health professionals who optimize use of medication for the benefit of the patients. Meets educational needs of patients, healthcare team, students and residents. Ensures adherence to regulatory and fiscal requirements.

Generic Scope

Provides direct supervision typically to professionals or skilled technical employees. Functions as advisor to unit and administration. Analyzes and resolves problems, interprets policies (e.g., fiscal management, HR, contracts and grants, resource management in defined areas) and demonstrates solid subject matter knowledge. Exercises judgment within defined procedures and policies to determine appropriate action. Supervises staff to assure accountability and stewardship of department resources (operational, financial, and human) in compliance with departmental goals and objectives.

Custom Scope

Provides supervision to pharmacists. May also include supervision of technical support staff. Responsible for assigned staff scheduling, timekeeping, training, goal setting, and performance evaluations.

Clinical Practice and Drug Therapy Management

Assesses drug therapy

• Determines and obtains the information needed to assess medication utilization as a whole within the hospital and as needed on a case by case basis.   

• Understands the pathophysiology of commonly encountered disease states and comprehends the medical management of the patient.   

• Determines if prescribed drug therapy is appropriate for patients and in compliance with all-applicable laws, regulations and policies. 

• Optimizes drug selection giving appropriate consideration to therapeutic efficacy, safety and cost effectiveness.  

• Recommends appropriate changes in drug therapy to achieve the desired therapeutic response in [specialty] area 

• Works with patient/patient’s representative, prescriber and other health care professionals to identify and resolve problems that arise with patient’s therapy (e.g. ADR’s, financial barrier, knowledge deficits). 

• Develops, performs and oversees, in collaboration with the Manager, medication utilization evaluations  

• Able to fulfill all duties of a Staff II & Lead Pharmacist

Qualifications

REQUIRED QUALIFICATIONS

• Doctor of Pharmacy Degree (PharmD) 
• PGY1 Pharmacy Residency, or equivalent experience 
• Three (3) years of experience as a licensed pharmacist in a hospital pharmacy or an equivalent combination of education and experience. 
• Demonstrates subject matter knowledge of pharmaceutical theory and industry best practices in optimizing the use of medication to treat patients. Stays updated on pharmacy regulations and developments in research and design. Ability to apply principles to an acute care setting. 
• Knowledge of relevant Pharmacy Department, medical center, and UC policies and protocols including medication storage, safety, and patient privacy and treatment. Knowledge of all pharmacy federal and state laws and regulations. 
• Demonstrated leadership skills in training and guiding employees in clinical service lines.   Motivates and inspires staff to improve quality and services. 
• Demonstrated ability to prioritize effectively to meet deadlines in a complex, challenging environment. 
• Ability to organize assigned staff and unit in an efficient and effective manner. Ability to apply policy knowledge to identify productivity inefficiencies and compliance risks. 
• Ability to develop and maintain cooperative working relationships with peers, patients, multidisciplinary team members, senior management, and assigned staff. 
• Skills to evaluate issues and identify solutions within defined procedures and policies. Demonstrated ability to solve patient problems and suggest solutions through critical thinking process.
