The Health System Command Center Pharmacy Coordinator supports centralized pharmacy operations through a virtual command center model that serves multiple hospitals and ambulatory care settings across the health system.

This role assists pharmacists and clinical care teams by performing medication reconciliation, patient outreach, medication access coordination, and prior authorization support while ensuring accurate medication management during care transitions.

The coordinator also supports Hospital-at-Home programs, facilitating medication coordination, patient communication, and medication access services to ensure safe medication use outside the hospital environment.

This position functions as part of a remote pharmacy services team designed to improve medication safety, patient outcomes, and care continuity across the health system.

Responsibilities

Medication Reconciliation Support
•    Conduct admission medication history collection through patient interviews, caregiver communication, and external pharmacy records.
•    Verify medication lists using available resources including:
o    EHR records
o    pharmacy dispensing records
o    prescription monitoring programs
o    outpatient pharmacy contacts
•    Document medication histories in the electronic health record for pharmacist review.
•    Identify discrepancies and escalate issues to pharmacists for resolution.

Hospital-at-Home Program Support
•    Assist with medication coordination for Hospital-at-Home patients.
•    Confirm medication availability and delivery for home-based patients.
•    Communicate with:
o    home infusion pharmacies
o    MSHS inpatient pharmacies
o    retail pharmacies
o    specialty pharmacies
•    Support medication delivery logistics and refill coordination.

Prior Authorization and Medication Access Support
•    Assist in obtaining prior authorizations for medications prescribed during hospitalization or at discharge.
•    Coordinate with:
o    insurance providers
o    pharmacy benefit managers (PBMs)
o    outpatient pharmacies
o    specialty pharmacy teams.
•    Track authorization status and notify pharmacists and care teams of approvals or denials.
•    Support medication access programs such as:
o    manufacturer assistance
o    copay assistance
o    medication affordability programs

Post-Discharge Follow-Up
•    Conduct post-discharge patient outreach under pharmacist supervision.
•    Confirm patients have:
o    received prescribed medications
o    understood medication instructions
o    scheduled follow-up appointments.
•    Identify medication-related issues and escalate to pharmacists when needed.
•    Document patient interactions in the EHR.

Medication Access and Prescription Coordination

• Confirm prescription transmission to outpatient pharmacies.
• Coordinate medication delivery or bedside delivery programs (Meds-to-Beds).
• Verify insurance coverage and copayment information for patients.
• Assist in resolving medication access barriers before discharge.

Command Center Operational Support
•    Monitor command center dashboards and patient queues for:
o    medication reconciliation tasks
o    discharge medication coordination
o    follow-up outreach assignments.
•    Maintain communication with site-based pharmacists and nursing teams.
•    Support surge operations and remote pharmacy coverage across multiple hospitals.

Qualifications

• High school diploma or equivalent required
• Experience supporting transitions of care programs
• Experience with Epic EHR
• Experience with prior authorization workflows
• Experience in telepharmacy or remote pharmacy services
• Experience with Hospital-at-Home programs
• Certified Pharmacy Technician (CPhT) through PTCB or NHA Preferred
• Registered Pharmacy Technician Preferred

Compensation Statement

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $34.00 - $45.95 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
